How to Schedule?

1. INSURANCE COVERAGE

ProVision Acupuncture is an out-of-network provider.

We welcome most patients with PPO insurance.

We do not accept HMO plans.

💡 Optional:
If your plan includes out-of-network benefits,

we can provide a Superbill (an itemized statement) upon request.

You may submit this to your insurance provider for possible reimbursement.

Insurance verification typically takes 1-3 business days.

If additional information is required, we will contact you.

2. SUBMIT CONSENT FORMS

Fill out the required forms online via our Online Consent Form page before your appointment.
Prefer to do it in person? No problem — you can also complete them at the clinic.

Visit our Booking Page to select your treatment, date, and time.

A small deposit may be required and will be applied toward your visit.
